Change of Information
In order to keep our records up to date as possible, please complete and return the form to the school office If any personal details have changed to a parent/guardian, student or emergency contact:
The school needs to be notified ASAP if any of the following have changed:
- Name
- Contact numbers
- Work and employment details
- Email address
- Languages studied external to St Martin's.
Please note that information can be updated through the Compass portal, by downloading and printing out this Change of Information form, or by filling in the online form below.
